			<content:encoded><![CDATA[<p>There is a lot of prospect in community-based mental health careers both in the state of Maryland and all over the country. This is because for years now, there has been a lot of emphasis on prevention and reduction of inpatient hospitalization for all illnesses, including mental illness. This might primarily have been intended for cost control, it has also facilitated quality and access. The second reason why career prospects in community mental health are many is that there is currently a severe shortage of mental health workers in all sectors. The 2007 Maryland Mental Health Workforce White Paper revealed that the number and complexity of mental health problems experienced by children and their families have increased over the past decade. It further said, “At least one in five children and youth, or 20%, experience a mental health disorder. The crisis of mental health in the United States is such that 75-80% of youth with mental health diagnoses receive no services, and services received are often inadequate”. Thirdly, there is inadequate diversity among the few mental health workforce. For example, 28% of Maryland population is of ethnic minority but only 12% of mental workforce is of ethnic minorities. Furthermore, there is an acute shortage of African American males in mental health workforce.</p>
<p> <strong>1. Outpatient Mental Health Clinics (OMHC)</strong></p>
<p>Outpatient mental health clinics provide therapy, counseling, medication management, social skills teaching, and case management services to individuals with severe and chronic mental health problems. Career prospects available in OMHC include:</p>
<p>Therapists and Counselors: New regulations require therapists and counselors in OMHC to have a minimum of a Masters degree and a license (such as LGSW, LCSW, LCSW-C, LGPC, LCPC, RNC, APRN/PMHN) in nursing, social work, psychology, counseling, or psychiatric rehabilitation. Also, an RN without a Masters degree but with an RNC from ANCC can be employed as a therapist. Salaries are very attractive.</p>
<p><strong>2. Psychiatric Rehabilitation Programs (PRP)</strong></p>
<p>PRP programs are an extension of the services provided to the patient in the OMHC. A PRP may stand alone or be an additional service to an OMHC. The purpose of PRP is to promote the rehabilitation, integration and improved quality of life for the patient at home, school, work and community. It aims at helping the patient to function at his or her optimum best in life. The counseling can be done at the Program office (onsite) or at the patient’s home (offsite). PRP counseling could be about a wide range of topics, including anger management skills, social skills, assertiveness skills, medication compliance, coping with symptoms, managing peer pressure, taking a bus, determining bus route, drug and alcohol, gang prevention, sex education, STD education, accessing community resources such as food stamps, affordable housing, bus pass, ID card, driver’s license, job search, preparing for job interview, keeping a job, improving attention in school, completing homework and school projects, respect of authority, etc.     </p>
<p>Even though a mere one-year work experience in a mental health setting or having an AA degree qualifies one to be a PRP counselor, PRP programs prefer to employ persons with a BS degree in any health or mental health related field such as nursing, social work, counseling, psychology and rehabilitation. PRP counselors are usually paid $14 or more per counseling session. Each client receives 2 to 8 counseling sessions per month.</p>
<p><strong>3. Expanded School-Based Mental Health (ESBMH)</strong></p>
<p>In addition to the school clinic, some schools also have an ESBMH clinic. A therapist assigned from an OMHC manages each of such clinics. Apart from providing therapy to troubled kids sent to the therapist’s office from the class or principal’s office, the therapist also serve as a resource person to the school staff regarding particular children, issues or topics related to mental health. </p>
<p><strong>4. Crisis Response Programs (BCRI, BCARS)</strong></p>
<p><strong>Mental health professionals are also needed in crisis centers where services are provided for anyone in mental health crisis. The two main centers in Baltimore are Baltimore Crisis Response, Inc. (BCRI) and Baltimore Child and Adolescent Response System (BCARS). <p><strong>BCARS</strong> is a mobile crisis response service that provides emergency contact with mental health professionals throughout the city. Dedicated crisis clinicians staff the program as part of a continuum of clinical care provided by the Catholic Charities.  The Johns Hopkins Division of Child and Adolescent Psychiatry provide psychiatric consultations to the program.  BCARS assists children and families facing psychiatric and psychosocial crises by providing hospital diversion and immediate intervention and respite. For information or assistance, please call the BCARS hotline (<strong>410) 752-2272</strong>. It is available 24-7. </p>
<p><strong>BCRI </strong>web site provided the following information: about what they do:</p>
<p>HOTLINE: The telephone crisis “hotline” (<strong>410-752-2272)</strong> is available 24 hours a day and is staffed by trained counselors who have the ability to provide information and referral to the network of human services in the Baltimore metropolitan area. The counselors also provide supportive counseling, dispatch emergency assistance and link callers with more intensive BCRI services.  In FY 2004 – 34,852 and FY 2005 – 30,257 calls were received on the Hotline.</p>
<p>MOBILE CRISIS TEAMS: Mobile crisis teams are comprised of mental health professionals including psychiatrists, social workers and nurses who can be dispatched to community locations to provide immediate assessment, intervention and treatment. Teams operate from 7:00am till midnight seven days per week. Currently the teams average over 2000 responses per year.</p>
<p>IN HOME SUPPORT: Persons experiencing a mental health crisis can often be maintained in the community through regular visits from the BCRI mobile crisis teams. An average of 350 people a year is cared for in this manner.</p>
<p>RESIDENTIAL CRISIS BEDS: Baltimore Crisis Response, Inc. operates 18 psychiatric crisis beds. Crisis beds are not new to Maryland. However, since its inception, BCRI has operated with an average length of stay of 4.5 days compared with the historical statewide average of 16.5 days.</p>
<p>PUBLIC EDUCATION AND TRAINING: BCRI provide public and professional education and training on a wide range of mental health related topics including: suicide prevention, crisis intervention, mental illness, and stigma.  Training has also been provided to members of the Baltimore City Police Negotiation Team, over 3,000 patrol officers, Housing Police and Sheriff’s officers. Through special grants and contracts, BCRI has provided training to Baltimore City Public School teachers and guidance counselors, clergy, 911 operators, shelter care staff and others.  Public education is also provided via a cable television program called “Mental Health Matters”.  This program provides practical information regarding mental health issues and community resources.  BCRI has also offered professional training conferences, workshops and symposia.</p>
<p>ADDICTIONS SERVICES: In response to the growing need for addictions treatment services BCRI has expanded and now provides a 10-day residential detoxification program for chemically addicted and dually diagnosed persons.  There are currently 16 beds operated for this purpose.</p>
<p><strong>5. Group Homes </strong></p>
<p>Direct care staff and counselors are needed in group homes to manage, care and support the residents in the areas of activities of daily living, behavior management, life progress, and community living. Employment preference is usually given to individuals who have a degree related to health or mental health. Salary rates are very attractive. New regulations now mandate each group home especially for children to be managed by a Program Administrator (PA) who must possess at least a BS degree in any field but preferably in a health or mental health related field. Program Administrators are very well paid, depending on their education and experience and the size and intensity of the group home. </p>
<p><strong>6. Private Practice</strong></p>
<p>There are a lot of prospects for licensed mental health professionals with at least a Masters degree to establish their own private practice. The practice could be in the area of clinical, research, educational, or consultancy.</p>

			<content:encoded><![CDATA[<p>MEDICAL / HEALTH INTERNSHIP IN NEPAL/ MEDICAL ELECTIVES IN NEPAL </p>
<p>Volunteer Society Nepal aims to mobilize medical students and volunteers with health backgrounds in rural places where people do not have access to any health institutes, as the government is not able to provide adequate health services. On the other hand, rural Nepali people are often reluctant to go to the hospital and other modern health services because of orthodox thinking, superstition and other cultural influences. Some of the Nepali people may visit a witchdoctor when they are suffering from an illness. Therefore, it is possible that they die without getting the benefit of modern medication. VSN is playing an active role in linking people in rural areas with volunteers from the west and health volunteers within the country who are really eager to serve the many unprivileged people in Nepal. </p>
<p>Volunteers will mainly be involved in following three activities:- </p>
<p>(1) Working in the Health Post/ Clinic/ Hospital in Nepal  Medical/ Premedical  students will be assigned to clinics, sub-health posts, hospitals and other health institutions. These are either run by the government or a local community in both rural and urban settings. Health students will work with health assistants, doctors, nurses or some combination within. Your daily work will include helping to perform the health staffs’ job. It will also involve researches of common diseases and health issues. Students will get the opportunity to learn about the basic medication system in Nepal. The work will be daily, 3 to 5 hours, 6 days a week. </p>
<p>The Health institutions provide health services to the poor and marginalized people in the community and volunteers will observe and practice following activities: <br />• Health Post Based Activities <br />1. General daily clinic <br />2. MCH <br />• Family planning (Temporary devices) <br />• Immunization <br />• Ante-natal and post –natal clinics <br />• Health education <br />• Health post based normal deliveries <br />• Immunization <br />3. Pharmacy <br />4. Simple surgeries <br />5. Community oral health Clinic <br />6. Nutritional Rehabilitation center <br />7. Health Post based nutrition follow up <br />8. TB Clinic <br />1. Maternal and Child Health Care <br />• Immunization <br />• Under 5 health screening <br />• Antenatal care <br />• Postnatal care <br />• Normal home and health post based delivery <br />• Identification and referral of high risk pregnancy </p>
<p>2. Family Planning Services <br />• DMPA <br />• COC Pills <br />• Norplant <br />• Condom distribution </p>
<p>3. General OPD Services <br />• General clinic <br />• Pharmacy <br />• Dressing and injection <br />• Minor surgery <br />4. TB dots clinic <br />• Identification <br />• Counseling <br />• Treatment <br />• Follow up </p>
<p>(2) Health Camps in Nepal <br />Part of volunteer’s program will be deployment to work in health camps for a predetermined amount of time: 1 day to 3 days. (Volunteers will work with their Nepali experience medical personnel. The students’ responsibilities are to distribute medicine and consult with citizens about health-care. <br />Through this Health camp, poor and marginalized people in the community will be provided general health services and students will observe and practice following activities: </p>
<p>Community Health Camp <br />• Examining patients <br />• Providing general medicines <br />• Identification and referral if needed <br />• Immunization <br />• Education on oral hygiene <br />• DMPA, COC Pills, Condom distribution <br />• Heath post based follow up </p>
<p>(3) Health and Sanitation Awareness <br />Volunteers will be mainly involved in teaching about proper health-care and sanitation and will work with youth clubs, women groups and other local organizations. In addition, they will provide HIV/Aids awareness, First-Aid information to the local people in rural villages and thereby providing them with a practical life-skill. Similar educational lectures will be given to children, by Groups, in government schools, lasting from several days to a week in duration. Again, the Groups’ major focus will be to educate and mobilize community groups who can then pass the information onto the community at large. </p>

			<content:encoded><![CDATA[<p>As most developed countries continue to move from a manufacturing-based economy to a service economy, and employee working patterns continue to evolve, health insurance coverage has become less stable. The service sector offers less access to health insurance than its manufacturing counterparts. Further, an increasing reliance on part-time and contract workers who are not eligible for coverage means fewer workers have access to employer-sponsored health insurance.</p>
<p>With health insurance premiums on the rise, many small employers cannot afford to offer health benefits to their employees. Companies that do offer health insurance, often require employees to contribute a larger share toward their coverage. As a result, an increasing number of people have opted not to take advantage of job-based health insurance because they cannot afford it.</p>
<p>How does being uninsured harm individuals and families?</p>
<p>*Lack of health insurance coverage compromises the health of the uninsured because they receive less preventive care, are diagnosed at more advanced disease stages, and once diagnosed, tend to receive less therapeutic care and have higher mortality rates than insured individuals.<br />
*Regardless of age, race, ethnicity, income or health status, uninsured children were much less likely to have received a well-child checkup within the past year. Research shows that almost 50 percent of children without health insurance coverage did not receive a checkup in 2003, almost twice the rate (26 percent) for children with health insurance coverage. <br />
*People without health insurance coverage end paying &#8220;up front&#8221; &#8212; before services will be rendered. When they are unable to pay the full medical bill in cash at the time of service, they can be turned away except in life-threatening circumstances. <br />
*About 20 percent of the uninsured (vs. 3 percent of those with coverage) say their usual source of care is the emergency room. <br />
*Studies estimate that the number of excess deaths among uninsured adults aged 25-64 is in the range of 18,000 a year. This mortality figure is more than the number of deaths from diabetes (17,500) within the same age group (10). <br />
*According to one another study, over a third of people without health insurance coverage have problems paying medical bills. The unpaid bills were substantial enough that many had been turned over to collection agencies &#8211; and nearly a quarter of the uninsured adults said they had changed their way of life significantly to pay medical bills. <br />
What additional costs are created by people without health insurance coverage?<br />
*Its estimated that America spends nearly $120 billion per year to provide uninsured residents with health services, often for preventable diseases or diseases that physicians could treat more efficiently with earlier diagnosis. <br />
*Hospitals provide about $35 billion worth of uncompensated care a year. <br />
*Another $38 billion is paid by private and public payers for health services for the uninsured and $26 billion is paid out-of-pocket by those who lack health insurance coverage. <br />
*People without health insurance coverage are 30 to 50 percent more likely to be hospitalized for an avoidable condition, with the average cost of an avoidable hospital stayed estimated to be about $3,300. <br />
*The increasing reliance of the uninsured on the emergency department has serious economic implications, since the cost of treating patients is higher in the emergency department than in other outpatient clinics and medical practices. </p>
<p>Getting Everyone Health Insurance Coverage Will Save A lot of Money.<br />
The impacts of living without health insurance coverage are severe. Many uninsured individuals postpone needed medical care which results in increased mortality and billions of dollars lost in productivity and increased expenses to the health care system. </p>
<p>There also exists a significant sense of vulnerability to the potential loss of health insurance which is shared by tens of millions of other people who have managed to retain health insurance coverage.</p>
<p>Every person should have health insurance coverage, participation should be mandatory, and everyone should have basic benefits because like we have seen above it&#8217;s benefits are awesome.</p>

More and more consumers are becoming aware that our current health care system is failing us in more ways than one and some kind of action is required, but get frightened or diverted by other&#8217;s negative perceptions and skepticism of natural health which is really based on misinformation and lack of understanding.</p>
<p>&#13;Most skepticism of alternative health modalities is fueled by the unfortunate fact that modern medicine tries to discredit alternative means of healing because it is much more profitable for them to use surgery and pharmaceuticals, not on any real grounds for concern. The topic of alternative health always brings out the doubting Thomas&#8217;s who scream quackery and voodoo. You may hear that they tried a holistic method without any success or that they had some weird side effect or even felt worse.</p>
<p>&#13;Here&#8217;s a couple important points to keep in mind. Their experience may be true but that does not mean it is true for everyone and there could be other reasons that contributed to their experience. We can very easily find another group of people who had great success and no side effects with the exact same treatment method they used.</p>
<p>&#13;This is because not everyone&#8217;s body responds to a particular treatment or healing approach in the exact same way. Even though you may share the same health condition with someone else, each of you has a unique body chemistry and genetics and the same disease can have a different process or impact from person to person. Each of these factors influences the results that will be achieved. In addition to that, someone with a liver, lymph system, endocrine system or other organ or system that doesn&#8217;t function optimally may have a very different response than someone who has healthier organs and systems.</p>
<p>&#13;Just because one treatment approach is ineffective for one person, does not meant it will be ineffective for you or others and that doesn&#8217;t mean that &#8220;all&#8221; holistic health care is ineffective. Results can be completely different from individual to individual. A lot of times it requires some trial and error to find what works best with your biochemistry and specific circumstances.</p>
<p>&#13;Additionally, this is true whether we are talking about a holistic health method or traditional pharmaceuticals. Regardless of whether we&#8217;re talking about natural remedies or prescription drugs, what is effective for one may not work at all for another, but overall holistic health is much more effective and a lot safer. We can find hundreds of thousands of people who have severe side effects, get worse, develop more serious health conditions or even die from prescription drugs. On the other hand, these events are rare within the field of natural health.</p>
<p>&#13;Holistic health or alternative health has actually been around longer than prescription medication or so called &#8220;modern&#8221; medicine. Plants and herbs have been used successfully to heal by many civilizations for centuries and most pharmaceutical drugs have their roots in herbal medicine. For example, the common and popular aspirin originated from an herb called white willow bark.</p>
<p>&#13;However, a crucial fact that many are not aware of is that just because something is natural does not mean it will automatically be free of side effects, complications or risks. Many people doctor themselves before first doing their homework and having a thorough understanding of the method they are using.</p>
<p>&#13;Herbs, plants, vitamins, minerals, or any alternative treatment method can be just as potent as a prescription and requires a great deal of education and research to be sure you achieve the desired results. They should be taken or performed as prescribed by a knowledgeable health care provider. For example: some holistic remedies that are effective for depression can have a negative effect on someone with Parkinson&#8217;s.</p>
<p>&#13;Too much or too little of a particular herb can be ineffective, while too much of a vitamin or mineral can be just as bad as not enough. There can be complications or interactions even within natural substances, as one herb or even treatment modality such as massage or acupuncture may counteract or rev up another treatment approach and vitamins and minerals work in conjunction with one another</p>
<p>&#13;Additionally, herbs and other natural substances can interact with prescription medication and reduce it&#8217;s effectiveness and vice versa. Its extremely important that you consult with a holistic health care provider and disclose all medications and conditions prior to medicating yourself.</p>
<p>&#13;Another important issue that may impact results of a holistic health remedy is the quality of the product or service you use. It&#8217;s vital to do your homework in this area as well and only use products and services from reputable companies. The natural health field is not immune to sales hype and scamming. You must be a diligent and informed consumer regardless of what path you follow.</p>
<p>&#13;Breaking free from the old way of thinking that conditions us to believe that drugs and surgery are the superior method of treatment can be frightening and difficult. It may seem foreign, threatening or strange at first, but when you get the facts about holistic health, you realize that it makes a lot of sense.</p>

But surprisingly, that&#8217;s not the number one health care issue confronting women today. In fact, heart disease kills more women each year than all forms of cancer combined. By knowing the most significant risks in women&#8217;s health care and what you can do to combat these health care issues, women of all ages can take a proactive approach to leading healthier lives. </p>

The #1 Women&#8217;s Health Care Issue: Heart Disease More than 489,000 American women lose their lives each year as a result of heart attacks, strokes, and other cardiovascular diseases. In fact, more women than men die from heart disease each year. Until recently, little research focused on women and heart disease, but the good news is that&#8217;s no longer the case. From research into the effects of hormone replacement therapy on heart health to studies of how cholesterol-lowering medications work in women, there&#8217;s an unprecedented increase in the amount of information available to women working to live with and ward off heart disease. </p>

A serum CA-125 blood test can detect certain forms of ovarian cancer, but the test is not accurate enough to be used as a routine screening tool. </p>
